Hospital at Home Service
Provides people over 16 years and their carers with support to enable them to remain in their own home (or Residential home) when suffering with an acute illness, or for palliative care, rather than being admitted to hospital. Patients are taken onto the service for a maximum of 14 days. Access to interpreters is available. Contact for more information.
